Accident and Incident Investigator Course

This course has been prepared to provide the participants with a methodical approach in order that they may be able to undertake a systemic safety investigation. It has been developed by the Safety Facilitators at the Civil Aviation Academy with the intention of also providing a resource to staff such that the organisation and the individual may benefit from a better understanding of the causes of accidents and incidents and the investigative analysis process.

This course and the associated guide/material outlines not only the investigative processes and tools but also reviews the main causal factors leading to these occurrences. It identifies major factors in safety deficiencies and suggests processes and guidelines such that agreed safety actions can be developed by an organisation or team.

Of significance are the critical roles of human performance to the safety process and the limitations this can place upon the process. An understanding of Human Factors and Human Error form part of the investigation course.

Although attendance at a course is indicative of management support for the safety investigation process, this must be ongoing and is achieved by strong management involvement and commitment to the process. Tools are given to enable investigators to better develop communication and reporting processes to achieve this level of interaction.

A change to the role of safety investigation now sees the prime objective being the prevention of recurrence and to advance safety. The activities of this course are not therefore designed to find fault with individuals.

Practical application of skills discussed and practiced is a major component of the course undertaken at the Civil Aviation Academy Australasia Pty Ltd.
